ম্যাক্সপ্রো মাপ্‌স হল রেনেটা পিএলসি প্রস্তুতকৃত ইসোমিপ্রাজল ৪০ মি.গ্রা. সমৃদ্ধ একটি প্রোটন পাম্প ইনহিবিটর। এমইউপিএস ট্যাবলেট আকারে প্রাপ্ত এই ঔষধ জিইআরডি, পেপটিক আলসার ও জলিঞ্জার ইলিশন সিনড্রোমের চিকিৎসায় ব্যবহৃত হয়।

• অত্যধিক অম্ল উৎপাদন রোগ

ইসোমিপ্রাজল পাকস্থলীর প্রোটন পাম্পে স্থায়ীভাবে বন্ধন তৈরি করে অম্ল নিঃসরণ কমায়। গবেষণায় দেখা গেছে, এটি রেসিমিক ওমিপ্রাজলের তুলনায় ৯৭% বেশি কার্যকরভাবে অম্ল নিয়ন্ত্রণ করে।

মাত্রাসূচীঃ

• প্রাপ্তবয়স্কদের ইসোফ্যাজাইটিস: ৪০ মিগ্রা দৈনিক ৪ সপ্তাহ
• শিশু (১২-১৮ বছর): ২০ মিগ্রা দৈনিক
• পাইলোরি চিকিৎসায়: ২০ মিগ্রা দিনে দুইবার + অ্যান্টিবায়োটিক

সাধারণ পার্শ্বপ্রতিক্রিয়ার মধ্যে মাথাব্যথা (১০-১৫% রোগী), পেটফাঁপা অন্তর্ভুক্ত। কিটোকোনাজল বা আয়রন সাপ্লিমেন্টের সাথে ব্যবহার এড়িয়ে চলুন। শিশুদের নাগালের বাইরে শুষ্ক স্থানে সংরক্ষণ করুন (২৫°সে.-এর নিচে)।

Erosive esophagitis-

• Adult (≥18 years): 40 mg once daily for 4 weeks.
• Children & adolescents (12-18 years): 40 mg once daily for 4 weeks.

Maintenance of healing of erosive esophagitis-

• Adult (≥18 years): 20 mg once daily.
• Children & adolescents (12-18 years): 20 mg once daily.

Risk reduction in NSAID associated gastric ulcer-

• Adult (≥18 years): 20 mg once daily for 4-8 weeks.

H. pylori eradication (Esomeprazole MUPS tablet with 1000 mg Amoxicillin and 500 mg Clarithromycin)-

• Adult (≥18 years): 20 mg twice daily for 7 days.
• Children & adolescents (12-18 years): 20 mg twice daily for 7 days.

Zollinger-Ellison syndrome and idiopathic hypersecretion-

• Adult (≥18 years): 40-80 mg twice daily.

Children 1-11 years:

• Erosive esophagitis: Weight <20 kg: 10 mg once daily for 8 weeks. Weight ≥20 kg: 10 mg or 20 mg once daily for 8 weeks
• Maintenance of healing of erosive esophagitis: 10 mg once daily

Children below the age of 1 year: Esomeprazole MUPS tablet is not approved for use in children younger than 1 year.

The most frequently occurring adverse events reported with Maxpro MUPS include headache, diarrhoea, nausea, flatulence, abdominal pain, constipation and dry mouth. There are no difference in types of related adverse events seen during maintenance treatment upto 12 months compared to short term treatment.

The manufacturer advises caution. It is not known if Esomeprazole or its metabolites appear in human breast milk.

Maxpro MUPS should be used carefully if the patient has severe liver dysfunction and severe renal impairment. Taking a proton pump inhibitor like Maxpro MUPS may slightly increase the risk of hip, wrist and spine fracture, particularly when it is taken over a period of more than one year.
